SPATIAL DISTRIBUTION OF HIV CASES, VISCERAL LEISHMANIASIS AND LEISHMANIASIS/HIV CO-INFECTION IN MARANHÃO
Abstract
The Human Immunodeficiency Virus (HIV), traditionally concentrated in urban centers, has been moving inland. Visceral Leishmaniasis (VL), in turn, linked to rural areas, has been urbanizing. The expansion of these geographic boundaries has resulted in an increase in cases of VL/HIV coinfection in Maranhão, Brazil. This study aimed to assess the incidence of HIV, VL, and HIV/VL coinfection in municipalities of Maranhão from 2007 to 2021. This ecological study used all new cases of HIV, VL, and VL/HIV coinfection reported to the National System of Injuries and Notification (SINAN) between 2007 and 2021. An exploratory spatial analysis was performed using choropleth maps of incidence per 100,000 inhabitants and calculation of the Local Moran Index to delimit incidence clusters. Distribution maps revealed a high incidence of VL and HIV spread throughout practically the entire state of Maranhão. VL/HIV co-infection was most frequent in the northwestern region of Maranhão. The spread of VL is attributed to urbanization, deforestation, and the spread of sandflies, while HIV, although concentrated on the coast, has reached the entire state. The increase in co-infection is due to the geographic crossover between the two diseases. HIV, VL, and co-infection are widely present throughout Maranhão. The study provides crucial spatial evidence to facilitate localized surveillance actions, directly contributing to the reduction of co-infection cases in the state.
