Abstract

Multisystem Inflammatory Syndrome in Children (MIS-C) is a severe post-infectious complication of COVID-19, characterized by hyperinflammation that mimics Kawasaki Disease and Toxic Shock Syndrome. Objective: To describe the clinical aspects and therapeutic management of a MIS-C case. Method: Case report of a patient treated at a teaching hospital in São Paulo, with data obtained from medical records. Case Description: A 10-year-old male patient was admitted with persistent fever, gastrointestinal symptoms, and hemodynamic instability. The investigation ruled out bacterial sepsis; laboratory tests showed elevated inflammatory markers and positive IgG serology for SARS-CoV-2 (with negative RT-PCR). Treatment was instituted with intravenous immunoglobulin (IVIG), corticosteroids, and intensive support. The patient progressed with significant clinical improvement and was discharged. Conclusion: MIS-C requires a high index of suspicion. Precise differentiation from other inflammatory syndromes and serological confirmation are crucial for assertive immunomodulatory treatment and a favorable prognosis.
