COMPARACIÓN ENTRE EL TRATAMIENTO CONSERVADOR Y QUIRÚRGICO EN EL SÍNDROME DE APNEA OBSTRUCTIVA DEL SUEÑO EN ADULTOS

El síndrome de apnea obstructiva del sueño (SAOS) es una condición altamente prevalente y subdiagnosticada en adultos, asociada a importantes repercusiones clínicas y funcionales, así como a una disminución de la calidad de vida, lo que hace fundamental una adecuada elección terapéutica. Ante la diversidad de enfoques disponibles, este estudio tuvo como objetivo comparar los resultados del tratamiento conservador y quirúrgico del SAOS en adultos, de acuerdo con evidencias recientes de la literatura. Se trata de una revisión sistemática realizada conforme a las directrices PRISMA, con búsquedas efectuadas en las bases de datos Web of Science, PubMed, Embase y ScienceDirect, en el período comprendido entre 2021 y 2025. Se incluyeron ensayos clínicos que evaluaron la efectividad de intervenciones conservadoras y quirúrgicas en pacientes adultos con SAOS. Al finalizar el proceso de selección, se incluyeron 28 estudios, que totalizaron 2.473 participantes. Los resultados indican que los tratamientos conservadores presentan beneficios consistentes en la mejora de la somnolencia diurna, la calidad del sueño y la calidad de vida, aunque con un impacto heterogéneo en la reducción del índice de apnea-hipopnea. En contraste, las intervenciones quirúrgicas demostraron mayor eficacia en la reducción objetiva de la gravedad del SAOS y mayores tasas de éxito clínico, especialmente en pacientes con alteraciones anatómicas específicas. Se concluye que ambas estrategias son eficaces y complementarias, y que la elección terapéutica debe ser individualizada, considerando la gravedad de la enfermedad, las características anatómicas, la adherencia al tratamiento y las preferencias del paciente.
