Introduction: Cholecystitis, an inflammation of the gallbladder, is common and clinically challenging. Ultrasound is crucial for diagnosis, while laparoscopic cholecystectomy is the gold standard of treatment, although conservative therapies are being explored. Objective: To review recent diagnostic and therapeutic approaches to cholecystitis, highlighting advances and challenges. Methods: A 15-year literature review was conducted, selecting relevant studies from recognized databases. Original studies and reviews addressing the diagnosis and treatment of cholecystitis in English, Portuguese or Spanish were included. Results and Discussion: Ultrasound is essential for early diagnosis. Laparoscopic cholecystectomy is effective, but conservative therapies are being investigated for high-risk patients. Technological advances offer new therapeutic options, such as robotic surgery and non-invasive therapies. Conclusion: An up-to-date understanding of the diagnostic and therapeutic approaches to cholecystitis improves clinical outcomes. Ultrasound is crucial for early diagnosis, while laparoscopic cholecystectomy remains the standard treatment. Conservative therapies are promising for selected patients, and technological advances offer new therapeutic options.
