Introduction: Vascular dementia, secondary to vascular brain lesions, is the second most common form of dementia. With an ageing population and an increase in vascular diseases, its burden on health systems is growing. This review addresses advances in understanding the mechanisms, diagnosis and management of vascular dementia. Objective: To explore the mechanisms underlying vascular dementia, examine advanced diagnostic methods and clinical challenges, discuss therapeutic interventions and management strategies, and highlight future perspectives and research challenges. Methods: A literature review was conducted, including original studies and reviews published without time limit. Inclusion and exclusion criteria were applied, and the search was carried out in various electronic databases. Discussion: Advances in understanding pathophysiology, vascular risk factors and their impact, and advanced diagnostic methods were highlighted. Therapeutic interventions, management strategies and palliative care were discussed. Conclusion: Vascular dementia is a global health challenge, requiring an integrated and individualized approach. Future research should target more specific therapies, while a multidisciplinary approach is crucial. This review summarizes recent advances and highlights the continued importance of research and clinical practice in vascular dementia.
