KETAMINE AS A THERAPEUTIC INTERVENTION IN PSYCHOTIC DEPRESSION AND TREATMENT-REFRACTORY DEPRESSION

According to the World Health Organization (WHO), depression will be the second leading cause of global disability by 2030. This disease is characterized by its heterogeneity and subtypes, which are classified by symptom variations. Objectives: to evaluate the use of ketamine as an intervention for severe psychotic depression and depression refractory to treatment. Materials and methods: This is an integrative review, in which the guiding question was “Is ketamine effective as a therapeutic intervention for psychotic depression and depression refractory to treatment?”. The search for articles was carried out in PubMed using the terms “depression refractory to treatment”, “psychotic depression” and “ketamine”, combined with each other by Boolean operators. Results and discussion: Ketamine is the first drug found to have an impact on treatment-resistant depression with immediate effect. It also appears to reduce the occurrence of suicidal thoughts and provides a rapid antidepressant effect with maximum efficacy reached after 24 hours. Short-term ketamine treatment may be a safe and effective option for patients with a history of psychosis or ongoing psychotic symptoms. Conclusion: New antidepressants are needed to treat depression that does not respond adequately to conventional treatment. Recent clinical trials show that ketamine is likely to be a revolutionary antidepressant, as it acts rapidly against suicidal ideation, as evidenced by decreased suicidal ideation, distress, and improvement in hallucinations.
