Abstract

Bilateral Tibial Hemimelia corresponds to a rare anomaly found in newborns with congenital absence of the tibia, and may be associated with several other malformations. In this research, we report a case of a newborn male, born at 38 weeks and 5 days of age, to a previously healthy mother, with no complications during the gestational period, with prenatal ultrasound showing a fetus with bilateral congenital clubfoot. During the orthopedic physical examination, the patient had both knees flexed and maintained at 30°, the distal third of the right lower limb was adducted, with the right foot completely inverted and supinated. The left foot is partially inverted and supinated. Bony prominence on the lateral aspect of the bilateral ankle joint was visualized.  The x-ray of the legs revealed a normal coxofemoral joint. The lower end of the normal femur and the patella present on both sides. In the left leg, the complete absence of the tibia was observed, and in the right leg, the absence of the tibia with a small cartilaginous anlage was observed. The fibula was present in both legs with no signs of duplication. Therefore, the objective of this research is to report a case of Bilateral Tibial Hemimelia, highlighting the low incidence of the anomaly compared to other cases and with an important impact on the child's development, making it necessary to carry out a multidisciplinary and specialized evaluation and management in relation to the different treatments.
