IMMUNOCOMPETENT PATIENT WITH DISSEMINATE CRYPTOCOCCOSIS: A CASE REPORT

Abstract

Cryptococcosis is a systemic fungal disease that affects humans and different species of domestic and wild animals and is considered rare in immunocompetent individuals. Objective: To conduct a case study of a patient without immunocompromise who was diagnosed with disseminated cryptococcosis. Methodology: Descriptive, retrospective study with collection of information from the patient's electronic medical record. Case study: Female, 39 years old, with no report of comorbidities or immunosuppression, negative rapid HIV test, neurological and respiratory symptoms for 3 months, with right hemicranial headache, emetic episodes, dizziness, loss of consciousness, loss of hearing and visual acuity and plergy in the lower limbs. Husband reports handling poultry at home (chickens). CSF with frequent cryptococcus. Treatment for disseminated cryptococcosis was started in addition to piperacillin and tazobactam for bronchoaspiration pneumonia. Chest tomography showing lesion in the middle lobe. Her clinical condition improved and she was discharged from the ICU and taken to the ward. The following day, she reported chest pain, ventilator-dependent, associated with dyspnea and mild cough, as well as tremors in the upper limbs and paresthesia in the lower limbs. Her sleep is not restful, with hyposomnia at night, and she accepts a soft diet well (assisted), with preserved physiological functions. She was discharged after her condition improved. She continues to receive care, now at the level of periodic consultations at the outpatient clinic. Conclusion: It is necessary to be careful when handling birds, especially pigeons and others that live in urban environments.
