THE CONTRIBUTION OF THE SPECIALIST NURSE IN THE MANAGEMENT OF PRESSURE INJURIES IN THE POST-HOSPITAL DISCHARGE PERIOD: A TELEHEALTH APPROACH – CASE REPORT

Abstract

This case report aims to describe the role of the enterostomal therapy nurse in the remote follow-up of an elderly male patient with a category 4 pressure injury in the sacral region after hospital discharge. Through telehealth, it was possible to conduct regular wound assessments, guide the healthcare team involved, as well as the patient and his family, and adjust the care plan according to clinical progress. The specialized remote approach proved essential to ensure continuity of care, prevent complications, and promote wound healing. It is concluded that remote follow-up by an enterostomal therapy nurse is an effective and safe strategy in home care, contributing to better clinical outcomes and reinforcing the importance of integrating telehealth into care practice.
