Long-term use of metformin, a common treatment for T2DM, is associated with vitamin B12 deficiency, requiring regular monitoring of vitamin levels in treated patients. Material and Methods: This is an integrative review of the literature on metformin-induced vitamin B12 deficiency in patients with type 2 diabetes, using PubMed articles from 2019 to 2024, with strict inclusion and exclusion criteria. Results and Discussion: Metformin is associated with vitamin B12 deficiency, impacting intestinal absorption of the vitamin, which can lead to complications such as neuropathy and anemia. Studies show that prolonged and high-dose use increases the risk of B12 deficiency in patients with type 2 diabetes. Conclusion: Metformin use is related to vitamin B12 deficiency, affecting vitamin absorption and causing complications such as neuropathy and anemia in patients with type 2 diabetes, especially in high doses and prolonged treatments.
