Type 2 Diabetes Mellitus (T2DM) is associated with hormonal dysfunctions, such as reduced free and bioavailable testosterone, especially in men. Testosterone replacement therapy (TRT) has emerged as a complementary strategy to improve glycemic control, body composition, and cardiovascular parameters in hypogonadal diabetic patients. This article reviews the main mechanisms and clinical evidence supporting the use of anabolic androgenic steroids as an adjuvant therapy in T2DM, highlighting benefits in insulin sensitivity, inflammatory profile, and muscle strength.
