The modern world has been associated, among other factors, with decreased sleep time and increased body fat. Sleep regulation is given by the action of the circadian system, regulated by the suprachiasmatic nucleus, which coordinates all metabolic and behavioral activities of individuals during the 24-hour period, including the secretion and action of melatonin, which acts as a chronobiotic, which , when out of balance, impacts on metabolic changes and eating behavior. The study aims to evaluate the effectiveness of melatonin supplementation as an adjuvant in the clinical treatment of obesity. This is a narrative bibliographic review, where data collection took place through Pubmed, Scielo and LILACS databases. Articles dated in the last 5 years were included, as well as information collected from the Brazilian Guidelines on Obesity (2016) and government websites such as the Ministry of Health and IBGE. Used the following DeCS: melatonin; nutrition; obesity; chrononutrition, in Portuguese and English. Melatonin has receptors in the peripheral adipose tissue, which results in its influence on the lipogenesis and lipolysis process, impacting the control of body weight. The action of exogenous melatonin on brown adipose tissue favors its hypertrophy, impacting thermogenesis regulation and balanced energy balance. Thus, its supplementation can be seen as an adjuvant therapy in the control of excess weight, as it favors the quality of sleep, resulting in reduced feeding time, control of the dietary circadian rhythm and management of insulin resistance in patients with excess body fat.
